The obligation
PCBUs must review and, where necessary, revise control measures when they may no longer be adequate — including after incidents, change, or when controls fail.
The Code requires PCBUs to review control measures regularly and when: controls may not be effective; a notifiable incident occurs; a WHS officer requests a review; or workers raise a concern. Reviews must assess whether the control measure is working as planned. Where a review reveals inadequacy, the PCBU must revise controls in accordance with the hierarchy. Workers must be consulted in the review process.
